Description
The contractor shall provide Ground Ambulance Services at Fort Polk, LA in accordance with the Performance Work Statement included in the SF1449. All offerors shall provide the following items: Please complete the SF1449: Blocks 17 (to include the CAGE code and the Unique Entity ID Number) and 30 A - C. Please provide a statement that the offeror will hold its price firm for at least 90 days from the due date for receipt of proposals, subject to any changes that may occur as a result of discussions (if applicable).
