AMENDMENT 00001 7 SEP 2026 All EAL request for the site visit must be submitted to the CO and CS listed in the contract information. 55 CONS will require the following information for each person wishing to attend the site vist: Legal Name of Guest: (Last, First, MI) Sex: Male (M) Female (F) DOB: (YYYYMMDD) Drivers License# & State or FNV Passport# or Immigration ID#: Citizenship (Country): Basketball court 4 is severely damaged, the protective coating peeling and stripping in many places, leaving the court unsafe for play and PFA execution. Provide necessary labor to resurfacing, retrofit the existing court with in floor net sleeves. SITE VISIT A site visit will take place on Monday, 14 September at 1300 CST. An EAL form will need to be requested from the POCs listed on this solicitation.
