Description
This solicitation will result in a Blanket Purchase Agreement with a 5-year ordering period. The Contractor shall provide health care support for Blanchfield Army Community Hospital. The contractor shall perform Audiology evaluations for eligible Department of the Army civilian employees from Rock Island Arsenal, Illinois (RIA) referred from the Occupational Health Nurse, RIA. The contractor shall provide the contract services at the contractors office, which must be located within a 60-mile radius of RIA as a required condition for effecting the contract services.
