General Information (REQUEST FOR INFORMATION/ SOURCES SOUGHT ONLY) 1. PROGRAM OBJECTIVE: The G.V. Sonny Montgomery VA Medical Center requires the following services to be provided in a private hospital, office, or clinic environment to Veterans, primarily residing in but not limited to the counties of Lowndes, Monroe, Oktibbeha, Clay, and Noxubee County, MS. 2. SCOPE: This site is classified as a Primary Care Community Based Outpatient Clinic CBOC offers both medical and mental health care and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide both primary care and mental health services. **Mental Health services will be provided onsite by VA staff, and or by VA staff via Telehealth. 3.
